Care Plan Team


Resident care follows a plan created by the resident, family and the Bethany Care Planning Team. Resident's strengths and needs are considered. The Team consists of nurses, a social worker, dietary manager, activities coordinator and the therapy director. They review the doctor's orders for each resident, nursing care, family and social strengths, diet and fluid needs, daily activities and interests, and therapy interventions. All of these are considered holistically in order to plan the overall care, daily routine and desires of the resident. Family members contribute helpful information on the history, interests and concerns of the resident. The resident is encouraged to speak about his or her goals for their stay at Bethany, including choices for their daily routine.

The Care Planning Team meets every 90 days, or more often if needed, to review the resident's plan, make changes and gather more information. Team members compare notes on progress with the resident, family and team members. The resident's doctor may provide information as well as specialists and other members of the Bethany staff. Everyone is on the Team and part of the Bethany family. The whole Team focuses their efforts on the resident.

Family members are notified in advance of the care planning meetings. They may pass along information and requests at any time. However, if the family member lives out of the area, a phone conference can be arranged. Unit Care Managers, who are the nurses in charge of resident care, may be contacted at any time to pass along information or ask questions.
